Ozempic and the Risk of NAION

In August 2024, a groundbreaking study published in JAMA Ophthalmology identified a significantly higher rate of NAION among people taking semaglutide (the active ingredient in Ozempic and Wegovy) medications compared to those on non-semiglutide diabetes medications.  

Another study published by Diabetes, Obesity & Metabolism in June 2025 further supported this link, concluding that the five-year risk of developing NAION doubled in patients using GLP-1 drugs compared to non-users.

This phenomenon is known as early worsening of diabetic retinopathy (EWDR) and has been observed in clinical trials of GLP-1 drugs. When blood sugar levels drop quickly, stress to retinal blood vessels can occur, leading to bleeding, swelling, or scarring. For patients with pre-existing mild or moderate DR, the effects of GLP-1 drugs could significantly worsen this condition.

Ozempic and the Risk of DR

According to the FDA Adverse Event Reporting System, Ozempic and other GLP-1 drugs have been associated with a significantly higher number of diabetic retinopathy complaints compared to other diabetes medications. In the SUSTAIN-6 clinical trial from 2016, patients taking GLP-1 drugs were nearly 1.5 times more likely to develop retinopathy complications than those in the placebo group. This increased risk has led researchers and regulators to urge caution for patients with existing eye disease.

Neovascular Age-Related Macular Degeneration (nAMD)

Another serious eye condition being linked to Ozempic, Wegovy, and other GLP-1 drugs is neovascular age-related macular degeneration (nAMD). This progressive disease can lead to permanent central vision loss. 

nAMD is the more aggressive (wet) form of age-related macular degeneration. It occurs when abnormal blood vessels grow beneath the retina and leak fluid or blood, damaging the central part of the retina responsible for sharp, detailed vision.  

Ozempic and the Risk of nAMD 

A recent article in JAMA Ophthamology (June 2025) suggests that vascular changes triggered by GLP-1 activity may trigger or worsen nAMD in some patients, especially those who are older, diabetic, or already at risk for macular degeneration.   • Distortion of straight lines (metamorphopsia)
  • Blurred or fuzzy central vision 
  • Dark or empty areas in the center of vision
  • Difficulty seeing fine detail, even with glasses
  • Sudden worsening of visual acuity
